Loaner Policies revisited!

Almost 2 years ago there was a hot discussion on writing loaner policies. I was wondering how everyone made out... and if anyone had a sample policy they would be willing to share? I have read the "Paper on Loaner Instrumentation " and it was very simple and easy to follow, I am just having a terrible time getting started.

We have just gotten a new Director of Surgical Services and she started her career many years ago as a Surgical Tech. She is extremely approachable, and they (OR) have just started updating their policy and procedure manual, and we feel now is the time to pounce!

Any help ASAP is greatly appreciated!!
