When a sterile set is opened and a dirty instrument is found - what is your written discipline process/standard?

We have OR save the paper that the tech signed with the tray and then they get a doc form - written down date and time they were talked to . After that a verbal, written, then they can be fired or you could remove them from instruments and take away thier pay for being a tech. Our disciplinary actions are in the handbook so we follow them. Document-Document-Document
